Healthcare Transformation Awards 2019 winners announced

Congratulations to all the finalists, highly commended and winners of the Healthcare Transformation Awards, held at the Health+Care Show 2019 last week.

The Awards were presented by NHSCC chief executive, Julie Wood, and co-chair Dr Graham Jackson. They celebrate the fantastic work being carried out across the country. We were delighted to see many of our members among the finalists, as well as so many projects showing collaboration across systems.

Highly Commended: The Kent and Medway Partnership – made up of Dartford and Gravesham NHS Trust, Medway NHS Foundation Trust, Medway CCG, and

Innovation in the care of long term conditions

WINNER: East and North Hertfordshire Palliative and End of Life Steering Group

Highly Commended: Islington GP Federation Quality Improvement Support Team

Innovation in improving outcomes and reducing variation

WINNER: NHS RightCare

Highly Commended: Norfolk and Waveney STP

Innovation in primary care:

WINNER:  NHS South Sefton CCG and NHS Southport and Formby CCG for Trans Health Sefton – a unified approach to gender care.

Highly Commended: Parchmore Medical Centre

Innovation in mental health care

WINNER: Lincolnshire Partnership NHS Foundation Trust and Lincolnshire County Council for their CAMHS Crisis and Home Treatment Service.

Highly Commended: Islington GP Federation and Camden & Islington Mental Health Trust.
